General Requests

(>) Contact the Help Desk
Have a problem or a question? Submit your issue to the University Computer Help Desk and a support technician will contact you.
(>) Preferred Name Request
Faculty and staff may use this form to request a preferred name. Students may request a preferred name in person at the Student Services Referral Center in Moulton 107.
(>) Listserv Request
Request a listserv to use with your class, department, or organization on campus. Use the listserv to send announcements or for group email discussions. For more information, see 1139: Requesting an email listserv.

ADILSTU Requests

Active Directory (AD) provides a variety of network services on campus, such as directory services and authentication.

(>) AD Guest Account Request
AD guest accounts are primarily intended for conferences or other gatherings where attendees do not have ULID accounts, but need access to computers.

Exchange Requests

Exchange is for faculty and staff only. Use these forms to request new Exchange accounts and distribution lists on the CISS back-end server.

(>) Distribution List Request
A distribution list is an Exchange mailing list used for notification purposes. Distrubution lists must comply with the Mass Electronic Communication Policy.
(>) Location and Equipment Request
Add resources to Exchange. You can add locations, like conference rooms, or equipment, like laptop computers.
(>) Exchange Account Request
Request access to Exchange for yourself. You can use Exchange for calendar only, or you may use it for both calendar and email.
(>) Student Worker Exchange Account Request
Request access to Exchange for a student worker in your employ. Student workers may use Exchange for calendar and work-related email.
(>) Other Exchange Requests

Media Requests

Media resources are available to faculty and staff who want to make use of audio and video content.

(>) Media Streaming Server Request
Use this form to request access to the media streaming server so that you can add streaming audio and streaming video to your website or blog.

Residence Hall Requests for Telecommunications & Networking

Students living in the campus residence halls can use these forms to make requests and report issues with ResNet and cable TV service.

(>) P2P Exception Request
Request to allow your ResNet connection access to peer-to-peer (P2P). You must use P2P for legal purposes only, consistent with Illinois State University's Appropriate Use Policy.
(>) Report a Broken ResNet Port
Use this form to report a physically damaged ResNet Ethernet port.
(>) Report Residence Hall Cable TV Problems
Use this form to report problems with cable TV in the campus residence halls. You must be connected to a computer on ResNet to use this form.
(>) Report Phone Problems
Use this form to report problems with a university phone. The information you provide will be used by Telecommunications & Networking to investigate and resolve your issue.

Non-Residence Hall Requests for Telecommunications & Networking

Telecommunications & Networking handles phone and network connectivity on campus.

(>) Report a Broken Data Port
Use this form to report a physically damaged Ethernet port. This form can be used by students, faculty, and staff.
(>) Report Phone Problems
Use this form to report problems with a university phone. The information you provide will be used by Telecommunications & Networking to investigate and resolve your issue.

ULID Requests

Everyone at Illinois State University has a University Logon ID.

(>) ULID Change Request
Your ULID is derived from your legal name as it appears on record with Illinois State University. If your legal name has changed or you find your ULID offensive, submit a ULID Change Request.
